I know this topic has been discussed a million times before, but I'm sure I've had transparent redirection of outgoing port 80, to an internal host on the same subnet, with a rule like (pseudo):

Redirect outgoing port 80 to <proxy_host> for ALL except <proxy_host>

This was years ago, so it would've been on pfSense 1.3.

I'm wondering what the solution for both 1.3 and 2.0 would be.

Did you tried to use squid package with transparent proxy option?

A rdr rule(firewall -> nat) the way you want could be done setting:

Source: not proxy ip
source port any
destination any
destination port 80
Redirect target IP proxy ip
