I suggest to focus on 3372. Make sure your USB port is capable to provide enough current, 3272 requires more than 3372.
In some cases those modems were working better when connected via relatively short (~15-20cm) extension cable instead of being plugged into the port directly.

I'm +1 for testing on a non-virtualized install.

There are some topics on this modem, please take a look:
Worth to search for "7455" through the forum.

I would start from checking what ports do you actually have open or enabled, check with at!usbcomp?
Then check the ports /dev/cuaU you see on your system to find a port which responds to AT commands.

Please correct me if I'm wrong - you have an operator branded modem and you already loaded universal/generic firmware?
Could you please show the result of AT^NVRD=50071

Prior to that you need to unlock some commands with AT^DATALOCK="v201 code"

Good point, worth to check. As I recall it was 2 different hw mods of E352, each with it's own fw.

Try to configure the modem part from scratch, i.e. delete everything, reboot, then check and configure.
Changing name means re-connect for me. I'm not FreeBSD expert, but in Linux the fixed name assignment could be achieved by udev rules, here you will need something similar. Worth to try devd.conf ?

was related to a modem itself, not the box it is plugged in
Here are the basics:

M0 or M Speaker off, completely silent during dialing M3 is also common, but different on many brands
M1 Speaker on until remote carrier detected (i.e. until the other modem is heard)
M2 Speaker always on (data sounds are heard after CONNECT)

BTW, E352 is not an "LTE-Modem".

Please keep in mind that modem support in pfSense is very limited, there is no support for modern NCM or MBIM or QMI protocols.
GUI support is limited to PPP, using HiLink modems (where USB modem is a router) is possible but requires some work.

What is in the log?
With UPS not connected I see the same "Failed to retrieve status" on UPS Status page and lots of messages from upsd and upsmon in the log.
What is the part number of the cable you have? Something like 940-XXXX
Do you have the same software (NUT) on your web server where you tested your UPS?

Remove all the rules you added specifically for your phone(s), remove Siproxd then speak to your SIP provider.

Not related to pfSense yet.
Remove all your customization - Sipproxy, outboundproxy, etc., then speak to your provider.
If you want to find a root cause yourself - look into your SIP traffic on both LAN and WAN.
